Evolving Nurse provides services from skilled nursing to homecare needs. Skilled Nursing is the first level of service offered. Our skilled team of healthcare professionals consists of Advanced Practice Nurses, Registered Nurses, Licensed Practical Nurses and Home Health Aides. Depending on the patient needs, we can arrange and provide an array of skilled nursing services to assist in improving you or your loved one quality of health. 

We offer the best home health care wherever you live in Cuyahoga County. Our Registered Nurses will work with your schedule to meet your healthcare needs. We have flexible scheduling to allow you to manage your day in a healthy way. Our nurses coordinate with your case manager and physician to implement an individualized care plan for each patient. 

We have a variety of services:

Personal Care, Sitter Care, Respite & Companionship, Homemaking, Cosmetic and Rehab services, Medication Management, Pediatric Care, Specialized Care, And more.

RN: Registered Nurses provide and support and serve as a liaison with the clients’ physician and case manager to coordinate healthcare needs. Provide 24-hour emergency telephone access and communication to primary caregiver as needed.

LPN: Licensed Practical Nurse services under the service of an RN; provides nursing care and/or treatment

HHA: Home Health Aides provide assistance with personal care such as bathing, dressing, grooming, meal preparation, incontinence care and transfers. Home Health Aides also perform a variety of housekeeping tasks and services to help restore or maintain the client’s health living environment. 

We offer private pay services for personal care, sitter care, respite care, cosmetic & rehab services and homemaking services.

Transportation services are only performed with the client’s own vehicle.
